If you would like to submit an application, you may download our Application for Admission and Information Release Consent forms and fax them to 828-350-9916.

To request a copy of these forms by mail, please email info@nextsteprecovery.com or call Executive Director Susan Stader at 828-350-9960.

To Submit Your Application Electronically

You may complete and submit your application electronically by using our Acrobat PDF form. You will need Acrobat Reader, which is available as a free download.

Right click on the link below to download the form or save the pdf file to your hard drive. Open the file and complete in Acrobat.

When finished, click the Submit Form button in the upper right corner of the page to email your completed form.
